Ever since Vem Miller was found in the vicinity of Donald Trump with a small arsenal of weapons, some of us have been wondering how he wound up being cut loose on his own recognizance. To be sure, plenty of investigations have taken place into all of these events. House hearings have concluded that these events marked “one of the greatest law enforcement failures” on record. So why has it been so hard to pin down the point of failure?
